Registered Nurse (RN) | Case Manager Location: Oakland, CA Agency: Lancesoft Inc Pay: $2,927 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/2/2026 About the Position Shift Day Shift: 5x8 hours Hours: 08:00 AM – 05:00 PM Schedule: Every other weekend required Float: May float to other UCSF facilities (within 75 mile radius) Responsibilities Manage pediatric patients’ care plans from admission through discharge Coordinate care with interdisciplinary teams (physicians, therapists, social workers, etc.) Perform discharge planning and arrange post?acute services Conduct utilization review and ensure appropriate level of care Work with families on education, resources, and transition of care Navigate complex cases in NICU/PICU and general pediatrics Document all case management activities in the hospital’s charting system Ensure safe, timely, and effective patient transitions Requirements Job Requirements & Qualifications Licenses & Certifications CA RN License (Required) AHA BLS (Required) COVID?19 Vaccine Booster required (Medical/Religious exemptions only) Flu Vaccine: Unknown Experience Requirements 2 years Case Management experience (Required) Discharge planning experience required Travel nursing experience: Required (within the last 12 months) Pediatric Case Management experience: Required NICU / PICU experience: Required Trauma Level II experience: Required Trauma Level I experience: Preferred Community Hospital experience: Preferred Teaching Hospital experience: Preferred Education BSN required Master’s in Nursing or Healthcare-related field: Preferred
